Forward a written purchase order to Teleflex Medical. The purchase order should include a purchase order number, billing and shipping instructions, terms, product numbers and description, quantity, price, and an authorized signature. Payment terms, ship mode, INCO terms.

By Fax

Place a phone or fax order by contacting Teleflex Medical’s Customer Service Department. Please provide a purchase order number, billing and shipping instructions, product numbers and description, quantity, and price. Payment terms, ship mode, INCO terms.
