All returns must be accompanied by a Returned Goods Authorization number, available by calling our Customer Service Department.
Call Customer Service to obtain a Returned Goods Authorization number.
The following information is needed to request authorization:
- Reason for return
- Date of purchase
- Invoice and PO number
- Product number and size
- Quantity to return
- Lot or serial number (if applicable)
All authorized returns must be made within ninety (90) days of the invoice date and accompanied by the appropriate purchase order number and Returned Goods Authorization number.
Allowable schedule for authorized return
- Products processed in error by Teleflex Medical will be credited in full
- Defective product will be credited in full
- Unless defective or shipped in error, any return accepted is subject to up to a 30% restocking fee
- All returned items are subject to Teleflex Medical’s inspection and acceptance
- Product must be shipped prepaid (if not processed in error by Teleflex)
Non-returnable, other than for a manufacturing defect
- Sterile products
- Products discontinued by Teleflex Medical
- Engraved instruments or instruments etched or labeled by hospital
- Products damaged beyond repair or refurbishment or not in saleable condition
- Implants (K-Wires, Steinmann Pins, Cerclage Wire) not currently excluded
- Custom, special or modified items
- Rubber goods
- Disposable products
- Merchandise damaged in shipment
- Products not in original packaging or in less than sales unit quantity
- Non-sterile products of a designated shelf life having less then 12 month's duration
